Hmm. Running this from a 32MB thumbdrive, on an x86 Vista. Will try some other boxes.

[Edit1] Either of you running NVIDIA? That's what this box is.
[Edit2] INI attached.

MAME crashes with your ini file by running from UI.
-----------------------------------------------------
Exception at EIP=0128B8E5: INTEGER DIVIDE BY ZERO
-----------------------------------------------------
EAX=00000001 EBX=00000000 ECX=00000000 EDX=00000000
ESI=00000000 EDI=00000000 EBP=0023FC78 ESP=0023FC48

I have a nvidia card.

[update] refreshspeed 1 causes the problem. All multi screen games crash via UI.

[update2] backtrace
Program received signal SIGFPE, Arithmetic exception.
