Is this with a pre-formatted backup memory with no save files? I'm well aware otherwise there are BRAM corruptions randomly happening (which implies that users should really do periodic backups of their data until a solution is found).

It's regular pce_cd_bram generated by MAME itself. I don't know how to format it even.

I tried to save the game, the game says "the backup memory is insufficient." and fails to save.

Is game confirmed to save on normal BRAM and not, say, require anything special like a セーブくん?

This page says "セーブくん is fully compatible with Memory Base 128. Any software required to store large amounts of data automatically detects the existence of the connection. 128 KB of data can be saved, and four AA batteries are used.",
セーブくん is released by Koei, Memory Base 128 is by NEC.
According to this page: http://bad.s33.xrea.com/pce/macrosslavepce.html
This game uses "127" backup memory (block?? kb??)

Set to confirmed, since Fujix was able to reproduce the problem.
Whether it actually is a bug is up to the devs to decide.

Regular BRAM has 2 KB, and yes I sorta managed to make it pass the saving by bumping the header size.
So yes, this definitely requires a goofy pad port memory device expansion.
It's otherwise okay to leave it here, I would have never guessed ...
